stock photo submission

Hello,
Would like to know what you guys/girls attach to an e-mail photo submission/request!
(this image submitted by request of... do not use... this image is for comp/viewing only...).
and where do you attach that form. folder READ ME FILES?

I include a PDF file named "Terms of Delivery".

I also make sure that every image has a "No Use" license written into its meta data, along with everything necessary to identify the image as mine.
